What Services Can YES Provide?
YES offers a variety of behavioral health and support services depending on your child’s needs. The Child and Family Team collaborates to decide the services that will best support the youth. The team facilitator helps bring everyone together and coordinates care so all providers and supports are working toward shared goals. Youth with private insurance may also contact a private provider to receive information about their covered care.
Some examples of services are:
- Case Management
- Counseling
- Crisis Intervention
- Respite

The Statewide Wraparound Referral Form is used to connect children and families to Wraparound services through Magellan Healthcare, the Idaho Behavioral Health Plan. Wraparound is a team-based planning process that brings together families, providers, and natural supports to create a personalized plan that helps meet the needs of youth who require more intensive supports.
This form collects important information about the child and family, including current concerns, strengths, existing supports, and any services already in place. It also includes details like school supports (IEP/504), health needs, and whether a CANS assessment has been completed, helping ensure the right level of care and coordination.
Families, providers, or other supports can complete this referral to request Wraparound services. Once submitted to a Wraparound Coordinator or agency, the information is used to determine next steps and begin building a team to support the child and family.
